Cluster Dementia:
Gardenia Cottage is the name of the cottage at Ginninderra Gardens Hostel for the dementia cluster program. The cottage environment allows for a stand alone area with separate access adjacent to an internal road into the facility.
The clients can also access the cottage through the main entry to the Hostel.
There are separate rooms with own ensuite bathrooms for each resident.
The room environment has a distinct separation of sleeping areas to a sitting area with access to a garden. The overall cottage environment is welcoming, light, airy and homelike, and is non threatening or institutional, instead it provides a pleasant ambience.
To continue the home-like theme the staff do not wear uniforms.
The programs aim is to provide a flexible, innovative respite care stream to support and maintain the carers in their caring role within the community.
Innovative approach is through a humanistic, wellness nursing model of care that focuses on personhood of resident and carers providing support and direction to meet their needs that will hopefully end in good outcomes for both.
With this aim in mind the program's goal is to provide care where the outcome is to assess, introduce and support strategies the carer has in place for the client to enable a safe return to the community.
To reach this goal the program's system is to use a discharge planning approach which because of it design needs to be flexible for the care recipient and the carer.
Admission:
Prior to admission the clients are referred to the organisation for an interview introduction and preview of the environment. During the interview the assessment is performed to establish the suitability of client for the program. If client is suitable for program and carer wishes to continue an agreement is entered into regarding the length of stay being offered, however this still remains flexible. The carer will be provided with an admission information package with details of the organisation. Admission date will be confirmed either at interview or within 48 hours when it has been necessary to perform further checks for clarification of resident's suitability for the program.
Some initial care planning is commenced from information gained at the interview.
Emergency:
Emergency admissions are admitted straight into facility following verbal discussions with ACT Carers to obtain some background to assist in planning for resident's needs.
Flexibility:
Flexibility of the program is to be able to provide short term 2-3 nights/weekends and longer periods of continuous respite up to 8 weeks or if required in exceptional circumstances 12weeks.
Medical Support:
Medical support for the program is provided by a GP sessional provider, they will also provide the link between the service and resident's GP to maintain communication and continuity of care.
Specialist support is provided by a consultant Geriatrician Adviser.
On Admission:
On the first day of admission a full and concise assessment process is begun.
The normal home routine is maintained as closely as possible to reduce risk of intuitionalism. The carers are encouraged to share in partnership with care, if they wish but there is no pressure to be involved.
Case Conferences:
To follow process of aim, to discharge resident's home, a case conference with the carer is attended in the first week of care. This case conference is to ascertain what the carer's needs are and if necessary at this time there may be an extension of length of stay offered.
Within another week to ten days a further case conference is held with carer, external service providers and GP input, either own GP if they wish to attend or from GP sessional provider. Further assessment is made of residents and carers needs to support them in the community and if required length of stay is extended.
In some cases there may be a need to undertake a further case conference it will depend on the complexity of the residents and carers needs.
Evaluation:
A full evaluation of the program is being performed. Carers are given a survey of the service from their perspective to provide feedback regarding the service. The organisation is attending to evaluative monitoring of case conference attendance and a monthly monitoring of occupancy is undertaken and an evaluation of the overall program is to be completed by Professor Alan Dunk from ANU.

Brindabella Gardens, Curtin
Brindabella Gardens has a 60 bed hostel and a 52 bed nursing home, these are divided into five care areas:
Nursing Home - General Area:
This unit accommodates twenty-four high care residents in three cottages, each of which has two twin and four single rooms with a sitting alcove. Residents use the large Brindabella Room for dining and entertainment.
Nursing Home - Illoura: Special Care Unit:
This unit provides secure care to 16 high care residents with advanced dementia and other special care needs. Admission and exit criteria apply. Accommodation is in twin and single room. The area has a separate lounge-dining area, an enclosed courtyard and sitting alcoves. Residents of this area also have the option of participating in the diversional activities conducted in the Brindabella Room.
Nursing Home - Victoria Shakespeare Cottage: Extra Service Unit:
This area is a private, self-contained unit offering a high standard of accommodation to twelve residents in large bedrooms with ensuites. Admission to this area requires the payment of a bond and extra daily fees. Residents of this area are also able to attend activities in the Brindabella Room.
Hostel - Memory Support Unit:
Located on the ground floor this closed area primarily accommodates 30 permanent residents, and one respite resident, requiring support because of memory problems and confusion. Each single room has an ensuite and access into an enclosed garden area. The unit has a small dining room called the Terrace Cafe for residents who are not able to use the main dining room, its own lounge called the Pavilion and a pleasant outdoor area with a covered veranda, paths and a barbeque.
Hostel - Upstairs:
The top floor of the facility accommodates 28 permanent residents, and one respite resident who have low to medium level care needs and whose primary requirement for care is related to their physical health or abilities. The area has a large open plan lounge room and dining room as well as a small tea-room. Facilities include a therapy/consulting room and a meeting room, for both staff and resident use. The single rooms have ensuites and a small private verandah.
